While LEDs (Light Emitting Diodes) are now widely recognized as emerging light sources for general illumination, it turns out that LED lighting can also enable a broad range of life-science-centric applications. The American Academy of Dermatology has identified a number of uses for LED lighting in skin-related therapies. LEDs are significant biologically because they can modify the function of mitochondria within cells. In the same way that plants use chlorophyll to convert sunlight into plant tissue, LEDs can trigger natural cellular photobiochemical reactions in the skin.
How do LEDs work?
LEDs offer a totally natural, non-ablative method for skin rejuvenation. The light from medical grade LEDs interacts with cells and stimulates them to produce new collagen and elastin. Studies confirm that the same LED energy can be used to affect collagen formation, which can be helpful in the treatment of scars. By manipulating the wavelengths, cells metabolism can be turned on and off, improving the blood flow, modulating inflammatory factors and proteins necessary to prolong the life of skin cells.

How many treatments will I need?
The number of treatments and the treatment times will vary depending on the problem being treated, the amount of surface area treated and the area treated. Treatments may last from 2 to 15 minutes. Since the results are progressive, you will notice more correction with each treatment. As full correction approaches, treatment intervals can be expanded to 3 – 6 month intervals. For scar prevention and healing, treatments are recommended at least 1 day before surgery, and 3 or more consecutive days after.
Will the treatments hurt?
No. The treatment is very relaxing, sometimes with a warm sensation. The delivery of the LED energy to the skin is completely pain free. The energy generated by the LEDs is gentle enough to treat all ethnic skin types.
What will I feel like after my treatment?
Most patients return to normal activities immediately. What patients like is that treatments are fast and they don’t have to miss any activities.
Can I have LED Skin Rejuvenation with other treatments?
Yes. It is highly complementary with other skin treatments such as Botox and dermal fillers, as well as non-ablative, ablative lasers and IPL treatments to improve and accelerate the healing process. Dermatologists and Plastic Surgeons worldwide are beginning to choose LEDs as one of their preferred treatments for very fine lines, light brown spots, acceleration of healing due to aesthetic treatments, but ultimately for scar prevention due to surgeries and many other conditions. Recent studies have shown that skin treated with LEDs before and after a cosmetic surgery tends to heal faster, showing amazing results for a better scarring, but also can prevent bad scarring, including hypertrophic scars and keloids. If using topical retinoids, it is necessary to stop them before and during the LED sessions. Before the treatments, you should tell the doctor of all the medications and creams that you are using.
Is LED therapy an option for my next visit to COSMED?
Absolutely, especially if you are having surgery or laser. LEDs will quicken the healing process, and at the same time will prevent/improve the scarring process. Canadians Travel to Mexico for Plastic Surgery, Seeking Quality Care, Better Value & a little Vacation Time

A tattoo used to be a permanent and irreversible adornment to one’s skin. However, in recent years, dermatologic surgeons have developed safe and effective techniques to successfully remove unwanted tattoos. Patients request removal of a tattoo for a variety of reasons — social, cultural, physical or for employment. Because each tattoo is unique, removal techniques must be tailored to suit each individual case. For instance, professionally applied tattoos tend to penetrate the deeper layers of the skin at uniform levels. This uniformity allows dermatologic surgeons to use techniques that remove broader areas of inked skin at the same depth. On the other hand, homemade or amateur tattoos are often applied with an uneven hand and their removal may be more difficult. Statistics show that about 24% of Americans between 18 and 50 are tattooed, and 15-20% of all people who got a tattoo regret it later.
Tattoos can be removed by a dermatologic surgeon on an outpatient basis with local anesthesia. The most common techniques used are:
The surgeon removes the tattoo by selectively treating the pigment colors with a high-intensity laser beam (Q-switched laser). Lasers have become the standard treatment because they offer a “bloodless,” low-risk, highly effective approach with minimal side effects. The type of laser used generally depends upon the pigment colors. In all cases, multiple treatments may be required.
The surgeon “sands” the skin, removing the surface and middle layers of the tattoo. The combination of surgical and dressing techniques helps to raise and absorb the tattoo inks.
The surgeon removes the tattoo with a scalpel and closes the wound with stitches. This technique proves highly effective in removing some tattoos and allows the surgeon to excise inked areas with great control.
ARE THERE SIDE EFFECTS OR COMPLICATIONS WITH LASER?
Side effects are generally minor (skin tenderness, inflammation, redness, crusts, scabs) but may include skin discoloration at the treatment site, infection of the tattoo site, lack of complete pigment removal or some scarring. A raised or thickened scar may appear three to six months after the tattoo is removed, especially with dermabrasion or surgery. The side effects are the reasons why laser tattoo removal is better achieved and with less side effects.
THE GOLD STANDARD: Q-SWITCHED Nd:YAG LASER NOW AVAILABLE AT COSMED
The Q-switched Nd:YAG laser system overcomes the obstacle of excessive melanin absorption and is used to remove gray, dark blue, and black pigment (1064nm), or red, orange and some yellow pigments (532nm).The clinical endpoint following laser treatment is whitening of the skin with occasional mild pinpoint bleeding. The specificity of our new laser targets only the ink or pigment on the skin, making it safe to use on all skin types. The light emitted by this laser will selectively gets absorbed by the pigment, heating and bursting just the pigment, which then later is reabsorbed and digested by cells from our body’s immune system, and finally gets eliminated via the lymphatic system.
HOW MANY LASER SESSIONS ARE REQUIRED?
There are some factors like: 1) skin color 2) location 3) tattoo pigment color 4) amount of ink/type of tattoo (amateur vs. professional) 5) scarring by the tattoo and 6) tattoo layering. These are the main considerations that influence in the number of sessions needed. Sessions are spaced every 4-8 weeks. When patients come to CosMed, we use the Kirby-Desai Scale to determine the number of sessions.
WHAT IS THE COST?
The cost is based on the size of the tattoo in cm2 and the number of sessions calculated by the dermatologist (1 cm2 = 0.155 sq in). If a package of the calculated laser sessions is paid in advance, we offer a 10% discount. Treatments start at $60 USD per session for tattoos 5cm2 (0.775 sq in) or less in size. Anyone who has traveled across the San Diego / Tijuana border knows how backed up traffic can get. Fortunately, a new medical tourism lane is now available. The new lane is already saving patients coming to Mexico for plastic surgery hours in border wait time.
According to a recent article posted on the Angeles Health web site, “A van transporting medical tourists from Las Vegas was the first vehicle to go through the new medical tourism lane in a special inaugural run that was held at noon on April 29 2012. An employee with Mexicali’s tourism board stationed at the border crossing took away the orange cones in front of the designated lane when the van pulled up, and then opened a yellow gate to let the van pass. As the medical lane was empty, the van rushed past the bumper-to-bumper traffic stuck in the two regular lanes.
Towards the end of the special lane, the medical tourists waited approximately 20 minutes to be inspected by a U.S. Customs and Border Protection agent.
“Unbelievable,” said Walt Michaels, a 62-year old patient on board the first vehicle who was going back to Las Vegas after seeing an eye doctor in Mexicali. “We just saved ourselves three hours.” See the full story here.

I just returned from the 2012 American Academy of Dermatology Annual Meeting. At the event, some new promissory treatments and technologies were introduced. Some of them show really nice results on the short term, while others lack long term benefits. As a safety and ethical policy to our CosMed patients, we only have devices and technology that have demonstrated beneficial results. We will not recommend a treatment just because it is the newest or trendiest thing on the market. We also know that just because a treatment is more expensive, it does not mean it is necessarily better. With that said, I would like to share some of the new and trending anti-aging technologies which everyone will be talking about soon. Ultherapy is a new type of non-surgical, non-invasive procedure for the face that uses ultrasound and the body’s own natural healing process to lift, tone, and tighten loose skin. It stimulates the deep structural support layers of the skin—including those typically addressed in a surgical facelift—without disturbing the surface of the skin. Results will unfold over the course of 2-3 months.
CoolSculpting, not really new but trendy, uses a targeted cooling process that kills the fat cells underneath the skin, literally freezing them to the point of elimination. Only fat cells are frozen. Your healthy skin cells remain healthy. Once crystalized, the fat cells die and are naturally eliminated from your body. You may start to see changes as quickly as three weeks after your treatment, and will experience the most dramatic results after two months. But your body will still flush out fat cells and continues doing so for up to four to six months after treatment.
In the same field of body contour and fat loss, Liposonix uses high-intensity focused ultrasound energy to permanently destroy targeted subcutaneous fat and deliver uniform. It gets rid of targeted fat around the waistline (abdomen and love handles) without surgery. The average waistline reduction after just a single one-hour treatment is about 1 inch (2.5 cm). Results are typically seen in 8 to 12 weeks (which is the time needed for the body to naturally process the treated fat). It is important to mention that it is not liposuction or surgery, is not a replacement for a healthy lifestyle and exercise, and is not a way to lose weight.
Zerona is a new body contouring diode laser procedure to effectively remove excess fat without the negative side effects associated with surgical methods. It utilizes cold laser technology which has been shown to emulsify fat and allow it to move to interstitial space. Although this technology was originally utilized to help assist tumescent liposuction procedures, it was found that the cold laser treatment alone helped patients to reduce inches from their waist, hip and thighs. There is no need to wear compression garments or bandage, and since there is no pain, there is no need for anesthesia.Some can see results after only a few treatment sessions as their clothes fit more loosely. Many actually see the difference after 4-6 sessions.
Fractional radiofrequency is a procedure that involves penetration of insulated microneedles into the dermis to deliver a high-tensioned radiofrequency pulse (heat) into the targeted tissue. This brings about the production of elastin and collagen fibers, making the skin firmer and healthier. This treatment could be more aggressive, invasive and with more side effects than classic radiofrequency (Thermage) which spares the microneedles, and obviously the pain.
New fractional lasers like Fraxel Dual, which uses a novel 1927 nm Thulium laser to target more superficial skin problems like dark spots, but mainly pre malignant lesions and with less downtime. A new option for patients suffering from hair loss is Fraxel re:store 1550. Even this laser is not really new, but its application on hair loss is. Recent studies showed that this laser could help in hair regrowth after 4-5 treatmens every 2 weeks, giving more hair density and thickness, both in women and men. Also the Clear + Brilliant laser which is a new anti-aging technology to treat and prevent signs of aging with no downtime, specially for younger patients, producing a visibly illuminated skin tone, a soft and smoother texture, an improved tone and radiance, and also a reduction in the appearance of pores. Also fractional lasers are now used as a medication delivery method instead of needles and syringes, especially for treating scars and nail fungal infections.
The trending in non invasive facial and neck procedures is now the combination of lasers and skin tightening devices, like the Thermage and Fraxel or “ThermaFrax”. Studies have demonstrated superior results that could last longer when combined together, especially in young patients, but this doesn’t means this could be a face or necklift substitute. Thermage delivers radiofrequency energy to the deeper dermis and subcutaneous layers of your skin to stimulate new collagen growth and tighten and contour the skin. Fraxel laser skin treatment resurfaces your outer skin layers, the dermis and epidermis, to smooth the skin surface and erase unwanted brown spots and wrinkles. This results in the enhancing of the skin tightening, texture, tone and the duration of the results. Thermage continues showing good results with just one session, as compared to other radiofrequency technologies in which more than one treatment are needed.
Talking about nail infections, the traditional vascular Nd:YAG 1064 nm laser has also demonstrated acceptable results for treating toenail fungal diseases in patients that are not candidates for oral medication or are only partially responding to it, with an average of 30-40% improvement. This was so new, that the regimen of frequency and dosage are still under investigation.
Finally, also robotics are getting into the medical aesthetic field, the ARTAS system is an interactive, computer assisted system utilizing image-guidance to enhance the quality of hair follicle harvesting. It has the potential to solve most of the technical challenges inherent in the manual follicular unit extraction technique. The system is operated under the direction of a physician, and includes an interactive, image-guided robotic arm, special imaging technologies, small dermal punches and a computer interface. After the system is positioned over the patient’s donor area of the scalp, it is capable of identifying and harvesting follicular units. The follicular units are stored until they are implanted into the patient’s recipient area using current manual techniques.

According to a recent article in Medscape, studies have shown that when Botulinum, also know as Botox, is used to relax frown lines, it can actually relieve major depression.
According to the March 2012 article, “a single injection of botulinum neurotoxin into the glabellar muscle of the forehead to relieve frown lines appears to lead to strong and sustained improvement of the depression.”
Tillmann Kruger, MD, Associate Professor in the Department of Psychiatry, Social Psychiatry and Psychotherapy at Hannover Medical School in Hannover, Germany, reported at the 20th European Congress of Psychiatry that these findings support the concept that facial musculature not only expresses mood states but also affects mood.
Dr. Kruger explained that frowning expresses negative emotions such as anger, fear, or sadness. A facial feedback hypothesis says that the frown itself reinforces negative emotions, with the implication that suppressing frowning will help to relieve the negative emotions.
“The theory is pretty old, and it says that many or most of the emotions we have develop somewhere in the brain, and some of them are expressed in your face, for example…and this is again received and sent back to the central nervous system by this proprioceptive feedback,” he told Medscape Medical News.
More about Botox
Botulinum toxin, otherwise known as Botox, is a popular non-surgical injection that can temporarily reduce or eliminate frown lines, forehead creases, crow’s feet, and other unwanted facial lines.
